Transition Research Project: Invitation to Participate

Dear YPHSIG members 

We would like to invite you to take part in a research project.  The purpose of the project is to better understand the nature of transitional care for young people with chronic health conditions in order to develop a clinical practice benchmarking tool.  We aim to explore young people’s and parents’/guardians’ views on how best to achieve continuity during transition from child to adult services and to explore the views of professionals on how best transition can be introduced and sustained.  We are looking for professionals who work with young people and have an interest in transition to take part in a consensus meeting. Our work is funded by The Burdett Trust. The research team includes:
